How to Access Your Patient Portal

Option 1: Access Through Our Website 

1. Visit the Associates in Dermatology website. https://www.traversedermatology.com/
    

2. Select Patient Resources.
    

3. Click Patient Portal (gray box located about halfway down the page). 

 

Option 2: Go Directly to the Patient Portal

Enter the following web address into your internet browser: 

traversedermatology.modmedapp.com
 

Then select Continue as Patient. 
 

Before You Can Log In 
 

Please provide your email address on your patient registration paperwork. You will receive an email containing a link to access your patient portal.
 

Logging In
 

Enter the following information:

• First Name

• Last Name

• Date of Birth
   

Verify Your Identity
 

For your security, a verification code will be sent to your text message or email. Enter the code to complete your login.
